I do not recall the name of the clinic where I donated, but it was situated on the southeast corner of Bloor Street and Jane Street in Toronto in a building now called Toronto West Professional Centre, 2425 Bloor St W, Toronto. I also do not recall if I was given a donor identification number, as it is more than 30 years ago.
I took some of my donations directly to the clinic, and others to St. Joseph's Hospital, now called St. Joseph's Health Centre.
I donated numerous times, and any children would have been born between 1980 and 1983.
I was a donor for Dr. AB Shewchuk,
Dr. Bernard L. Ludwig, and Dr. A. Eisen
of Toronto from October 1984 to June 1987.
