Dr. Jonas and Dr. Platock
Doctors Office
The old office location has since closed down in Arlington area of Jacksonville, FL. The old office location may have been 1201 Monument Rd,Suite 301, Jacksonville, FL 32225 but I am not certain as both Dr. Jonas and Dr. Platock have retired and the office may have had several different locations at that time.
